Medical Form and Medical Form Instructions  Forms must have been completed within the past 12 month.
Day Camp and Resident Camp - Part A & B required (No Doctor necessary)
Webelos Camp - Part A, B & C required (Part C will require a doctor's signature)
Suggestion: If possible fill in all info electronically except for date and signatures. Save the form in a location you will remember next year then Print the form to add signature and date. Every year you can merely update data if needed, print out form again add signatures and date.
