NEW MEDICAL FORM

2014: New version! The content is the similar and you can still type directly into the form then save and print.

Each Scout must complete this medical form (PDF, 632 KB) every year. The document includes Parts A, B, and C. Part C requires a doctor's signature and is necessary for participation in summer camp or any high adventure camp.
